Refer a Patient
Before an appointment can be scheduled, you will need a referral from your physician.
How it works
Your doctor will need to complete our referral form and fax it to our office to fax # 604-628-2419.
Once the referral is received, the clerical team will process the request and create a waitlist entry in our system as well as send a notification to the referring physician that we have received the referral and the status is pending.
The referral will be reviewed and triaged by the gastroenterologist who will decide the next steps in your management.
The referral should include all your demographic information, the reason for referral, your past medical history, family history, current health status, your current medication list, and all relevant investigations such as labwork, imaging reports, or other specialist reports. Incomplete referrals will be returned to the referring doctor’s office and may result in delay of your care.
